Two local Lake County FL Hospitals, Waterman & Leesburg, received an A and a C score respectively on a Hospital Safety Score report. You can read more how the scores were issued at the links below.
The scores were published nationally by Leapfrog Group, a national nonprofit that advocates for safer healthcare delivery. "Health analysts based hospital grades on 26 variables, including rates of infections, medication mix-ups, acquired injuries (including bedsores), and other preventable — and often fatal — conditions."

Scrolling down the pages below, you will find many details on how the score was developed, but they don't provide any place for a response from the ranked hospitals. Knowing the Hospitals somewhat from observation, I am guessing Waterman knew this study was coming, and implemented procedures early to ensure their safety methods were improved.

One thing to keep in mind, is that this is the first year these rankings have been published, the data could be two years old, and no matter what you see about US hospitals, the patient safety in foreign hospitals is a LOT worse. Only about 450 foreign hospitals have been accredited worldwide (outside the US) by the Joint Commission International Accreditation organization. http://www.jointcommissioninternational.org/
